The word ONLY is pointing to a senario among the answer options where autosomal dominant (and not the autosomal recessive) inheritance will be the single possibility, while other options have more than one possibility.

What would all children’s phenotype be, if both parents bear only diseased alleles (i.e. autosomal recessive)?

When I was solving this test I choose the second option - A. I couldn’t understand the condition completely, so it felt like an accidently right choosen answer. First I eliminated 1, 4, 5 and then chose what was more likely to be right. Thank you!!
